Does creatine make your body puffy?

Short answer

It's not puffiness. Weight rises 0.5-1 kg in the first week, but that water rose along with muscle, and the split between water inside and outside cells stayed the same.

  1. Does creatine make your body puffy?

'Creatine → water retention → puffy body' — does this formula hold?

A common claim says creatine makes your face and body puffy with 'water weight.' Plenty of people say the scale went up after they started. We checked, step by step, why weight goes up and where that extra water goes.

  2. Weight does go up

Lean mass rose 1.1 kg in 1 week

27 people took creatine for 1 week, about 21 g a day for a 70 kg (154 lb) person. Their lean mass (muscle and everything except fat) rose 1.1 kg, more than in the placebo group. Body water went from 40.4 kg to 41.2 kg, up 2%. The extra weight was water.

  3. So does that water pool under the skin?

The inside-outside water ratio stayed the same

In 32 people taking creatine for 28 days, weight and body water rose, but the split between water inside and outside cells did not change. In 27 people lifting for 8 weeks, muscle and water inside cells rose together, while water outside cells rose the same as with placebo.

  4. What about women?

No weight difference, but more water before the period

30 women took 20 g a day for 5 days. Their weight change was no different from placebo. But when they loaded in the days before their period, body water rose 0.83 L, and water outside cells rose 0.46 L.

  5. The weight came from 'loading'

At 2.3 g a day, weight and body water stayed put

An analysis that pooled 100 studies found body composition changes were clear during loading, the high-dose start, and nearly absent with small daily doses. 20 people taking about 2.3 g a day for 6 weeks saw no change in weight or body water, yet tired less across repeated sets (7-11%).

  6. The puffiness doesn't keep building

51 kg in week one, no gap after that

In a 2025 study of 63 people taking 5 g a day, lean mass rose 0.51 kg more than in non-users over the first 7 days. Over the next 12 weeks of training, both groups gained 2 kg each, with no difference. It went up once in week one and the gap never widened.

  7. Bottom line: it's water in muscle, not puffiness

A one-time bump in week one, on the muscle side

Creatine adds 0.5-1 kg in the first week, but that water rose along with muscle and did not change the inside-outside ratio. If the scale bothers you, 3 g a day with no loading works. In 28 days, muscle creatine rises the same 20% as with loading.
