Can exercise ease neck pain?

Short answer

Yes. For long-lasting neck pain, strength training, movement-control exercise and yoga all reduced pain, and neck-and-shoulder strength training worked in 20 weeks at 1 hour a week. Stretching alone made no difference.

    'Don't move it, rest it until it heals' — is that right?

    Neck pain from long hours of sitting is common. Some say rest it, others say exercise it. We checked whether exercise helps long-lasting neck pain, and which exercise and how much it takes.

  2. People who exercised had less pain

    Pooled results from 40 trials and 3,151 people

    An analysis that pooled 40 trials on long-lasting neck pain found three types of exercise — strength training, exercise that trains posture and movement control, and yoga or Pilates — reduced pain and daily-life limits compared with no exercise. None clearly beat the others.

  3. 68 trials reached the same conclusion

    Strength, movement control and yoga all reduced pain

    A 2023 analysis that pooled 68 trials also found all three types reduced pain compared with doing nothing. Yoga and Pilates cut pain a little more, and movement-control exercise cut neck-related daily limits a little more.

  4. Stretching alone changed nothing

    Neck and shoulder strength training did the work

    A Cochrane review of 27 trials and 2,485 people found strength training for the neck, shoulders and arms cut pain by a moderate to large amount right after the program. Stretching alone did not change pain or neck function.

  5. After 1 year

    Pain scores dropped from 5.0 to 3.4

    In a trial that followed 549 people for 1 year, those who started with neck pain went from 5.0 to 3.4 on a 0-9 pain scale with neck and shoulder strength training. The all-round exercise group dropped to 3.6, while the health-advice-only group did not change.

  6. How much is enough?

    1 hour a week worked, even split up

    For 20 weeks, 447 people did 1 hour a week of dumbbell front raises, side raises, reverse flies, shrugs and wrist extensions. The 256 with pain of 3 or more improved by 1.14-1.88 points more than the no-exercise group. Doing the hour at once or in 9 sessions of 7 minutes made no difference.

  7. Bottom line: strength training worked for neck pain
