Can fat freezing or liposuction replace exercise?

Short answer

No. The procedures reduced fat in one area, but metabolic markers stayed the same, removed fat returned to the belly within 1 year, and without exercise after liposuction, visceral fat rose 10%.

  1. Can fat freezing or liposuction replace exercise?

'Removing fat = losing it through exercise' — does this hold?

Many people want to lose belly fat with a procedure instead of exercise. We checked whether fat freezing, which kills fat cells with cold, and liposuction, which sucks fat out through a tube, give the same results as fat lost through exercise.

  2. How much does fat freezing remove?

    Pinchable fat thickness fell 15–29%

An analysis of 19 studies found fat freezing cut pinchable fat thickness by 14.67–28.5% on average, or 10.3–25.5% measured by ultrasound. An analysis that pooled 30 studies with 3,158 people found waist size 3.56 cm smaller than before treatment after 12 weeks.

  3. Does liposuction change what's inside?

Removing 10 kg of fat left metabolic markers unchanged

15 people had 9.1–10.5 kg of fat (18–19% of their total fat) removed by belly liposuction. After 10–12 weeks, insulin response, inflammation levels and heart disease risk markers such as blood pressure, blood sugar and cholesterol did not change.

  4. Does the removed fat come back?

After 1 year, the body fat difference was gone

In a trial that split 32 people into 14 who had liposuction and 18 who did not, body fat percentage fell 2.1% in the liposuction group at 6 weeks. By 1 year, the difference between groups was gone. The thighs stayed smaller, but fat built up again on the belly.

  5. Exercise made the difference

Only the no-exercise group gained 10% visceral fat

2 months after belly liposuction, 36 people were split into a group that exercised for 4 months and a group that did not. At 6 months, the no-exercise group had 10% more visceral fat (the fat packed around organs) and burned fewer calories per day. The exercise group did not gain visceral fat.

  6. Side effects of fat freezing

1 in 455 people saw the fat grow instead

After fat freezing, 49.5% had numbness, 44.5% redness and 30.5% swelling. An analysis that pooled 13,078 people found the treated fat grew larger instead in 0.22%, about 1 in 455 people.

  7. Exercise reduced visceral fat too

39 studies: whole-body, belly and visceral fat all fell

An analysis that pooled 39 studies with 617 people found high-intensity interval training reduced whole-body fat, belly fat and visceral fat. The procedures only removed fat under the skin, and no result showed them reducing visceral fat.

  8. Summary: procedures shape, exercise works on the whole body

    Procedures did not replace exercise

Fat freezing cut treated fat thickness by 15–29%, and liposuction removed 10 kg of fat. But metabolic markers stayed the same, fat returned to the belly within 1 year, and without exercise visceral fat rose 10%. Procedures reshape one area; exercise was what reduced visceral fat.
